After all, the placement of each implant dictates how your “replacement teeth” will appear and function. The more precise the result, the better your look and the easier the appliance is to maintain with your home care and with your hygienist. Furthermore, we feel that the integrity of your bone should be maintained, whenever possible, with bone grafts used to repair bone defects and fill extraction sockets

Most All-on-4® Type of Implant Therapy of procedures advocate bone reduction for surgical and restorative convenience and speed, rather than bone preservation, which is our preference, when possible. After all, one never knows when that “extra” bone would be needed in the future, and bone grafting preserves that option. Whenever you obtain an opinion about an All-on-4® Type of Implant Therapy of procedure (also known as “teeth in a day”, “immediate load implant therapy”, “same day implants”, and other terms), ask if that clinic or office would reduce or preserve your bone.

I was born with bad teeth. Not misshapen or misaligned, but weak. When I had my wisdom teeth removed in my early 20s, what I was told might be a 30 to 45 minute procedure was over in 10 minutes. As the dentist tried to extract them, the teeth simply broke into pieces and came out quite easily. At some point in my 20s, I realized that flossing was actually abrading and wearing down the surface of my teeth at and below the gumline and so I stopped flossing. That meant that receding gums and bone loss were added to the incremental tooth loss as my back teeth decayed and broke. By the time I hit my mid-50s, I had lost most of my back teeth, had two broken and visible front teeth, and badly receding gums. This was the disaster scenario Dr. Gerald Marlin faced when he first met me. With the help of sophisticated imaging, he was able to show and explain to me just how dire my situation was. I was hoping to have the All-on-Four implant full-arch denture treatment for both the upper and lower jaw. Unfortunately, as Dr. Marlin showed me, I didn’t have sufficient bone, especially on the upper jaw, to do the procedure. Undaunted, he came up with a plan to make my desired outcome possible. We would rebuild the bone, and then go from there. I quickly came to realize that the “new teeth in a day” promise made by the mass-marketed All-on-Four providers was, ultimately, a scam. Sure, there might be a very select number of individuals, with no complicating circumstances, who would be candidates for a one-day procedure. But even then, as Dr. Marlin explained to me, the typical move by such operations is to level-off the bone under the gums before sinking the implanted sockets. This means that, if further bone loss were to occur in the ensuing years, the patient would be out of luck, because so much bone would have been sacrificed in the initial “tabling” of the bone. Dr. Marlin convinced me that a much more gradual approach would give me the greatest chance of success. And that’s how it went. I had all my remaining teeth removed, bone grafts done, and was fitted with temporary, traditional, dentures all at the same time. With periodic appointments to check the success of the bone grafts, I waited for everything to be set for the sockets to be implanted. When the bone grafts had taken, I had the sockets implanted and a temporary set of screw-in appliances installed both top and bottom. Time was taken to make sure all the now non-moving parts were working correctly. Finally, the finalized upper and lower appliances were made and installed. Yes, the process took a year and a half. With numerous visits. And yet, thanks to Dr. Marlin’s generous discounts, it proved to cost really no more than the “new teeth in a day,” assembly-line type treatments which, I’m convinced, could NEVER have produced the results I’ve had. I now wake up every morning with a gorgeous smile, and one I don’t have to take out and put in a glass to soak overnight. Dr. Marlin is a true artisan and a true professional. And he has assembled a great group of individuals to help him do what he does. Which for me, was a miracle. I hope you will trust your dental health and aesthetics to this first-class team.
